Tadalafil is a Phosphodiesterase 5 enzyme inhibitor. It inhibits the enzyme phosphodiesterase 5 and thereby increasing the effect of cyclic GMP on the muscle that has a vasodilatory effect on the blood vessels. In simple words, it opens up the blood vessels for more blood flow to the particular organ. Tadalafil is used to treat erectile dysfunction and also pulmonary arterial hypertension. The doses for both the conditions vary and therefore causes different effects at different doses. Tadalafil comes as generic as well as with a brand name of Cialis for treating erectile dysfunction. For the treatment of pulmonary hypertension, it comes under the brand name of Adcirca. Cialis was originally produced by a joint venture of ICOS and Eli Lilly pharmaceutical company.

Tadalafil belongs to the same class of drugs as Sildenafil (Viagra), Vardenafil (Levitra), and Avanafil (Spedra). Tadalafil comes in doses of 2.5mg, 5mg, 10mg and 20mg. If it is taken prior to a sexual activity then a dose of 10mg may be taken, this dose can be increased up to 20mg based on individual tolerance and effectiveness. If Tadalafil is being used as one tablet every day for treating sexual dysfunction a starting dose of 2.5mg is taken. In this case, the drug remains in the blood for up to 32 hours and the drug levels continue to build up within blood giving the desired effect whenever required. One benefit of taking tadalafil daily is that the sexual activity is not planned around it and gives more room for spontaneity and fun.

Let’s compare Tadalafil with other medications and see what benefits it offers over other medications of its class, and why to choose it over others.

Why did Tadalafil come to market when Viagra (Sildenafil) was already present?

Viagra (sildenafil) was launched by Pfizer pharmaceutical company in 1998 to treat erectile dysfunction. The drug was hugely popular among men and almost revolutionised the treatment for E.D. Tadalafil under the brand name of Cialis was launched after 5 years of Viagra in 2003. Viagra’s success, there was a huge potential market for development of drug molecules that would treat erectile dysfunction. So far five oral medications have been developed for the treatment of erectile dysfunction and Tadalafil is one of them. For first ten years after the launch of Viagra, Pfizer owned the patent of the drug and only it could only be sold under its brand name. After the patent was over, generic Viagra as Sildenafil came to the market. Seeing the blockbuster sale of Viagra other pharmaceutical companies pounced upon the opportunity to develop more molecules like Viagra and enter in the potentially huge market of treatment of erectile dysfunction. All of these five medications have an almost similar mode of actions but still, there are certain differences between them that make them useful over one another. Depending upon the individual condition and disease state the drug options are also weighed against one another. Usually, all of the erectile dysfunction drugs are compared to Viagra because of its popularity, ease of use and instant effects.

But it is worth mentioning here that after its launch in 2003 Cialis has gained a popularity that is a step ahead of Viagra. One of the main reasons for its popularity is that it can be taken daily with minimum side effects. Unlike Viagra, it is not necessary to maintain a time frame within which it needs to be taken. We go into more detail about the reasons behind the launch of Tadalafil here.

Difference between Viagra (Sildenafil) and Tadalafil?

As we have already discussed that why Tadalafil came to market when Sildenafil was already present. Although the mode of action of the drugs is similar yet there are certain differences between Sildenafil and Tadalafil that sometimes make one popular over another.  Let’s see a few differences between them;

  • Onset of action and length of time in blood:

Tadalafil stays in the blood longer compared to Viagra. Its onset of action starts within 30 min of its consumption and it remains in the blood for up to 36 hours whereas Viagra remains for only 4 to 5 hours and its onset of action is 1 hour. If a drug has a longer stay in blood then its efficiency is also increased.

  • Daily dosage intervals and potency:

Tadalafil can be taken every day whereas Viagra cannot be taken every day and is usually taken just prior to a sexual activity. Viagra has a maximum dose of 100mg that can be taken safely. More than a dose of 100mg can cause harmful effects to manifest itself. Taking Viagra daily asian female giving pack of blue pillsdoesn’t cause it to accumulate within the tissue of the penis, and hence causing erections on demand. Unfortunately, the drug doesn’t work that way and therefore shouldn’t be taken if sex is not on the table. Accumulating it in blood without having intercourse results in unwanted side effects and a psychological dependency. It may very much be possible that a man starts feeling that taking Viagra daily makes him feel more manly or good about himself. It should always be kept in mind that these drugs have a potential for abuse if taken without a prescription or without the advice of a GP or a pharmacist. Viagra comes in doses of 25mg, 50mg, and 100mg. It is usually taken 30 min to one hour before coitus.

Tadalafil comes in doses of 2.5mg, 5mg, 10mg and a maximum dose of 25mg. A dose of 2.5mg can be taken daily at almost the same time of day irrespective of the sexual activity. This daily can be increased to 5mg based on individual efficiency and physical tolerance.

10mg is the dose that is taken just prior to the sexual activity. It can be increased to a dose of 25mg. Unlike Viagra, Tadalafil can be taken daily. It is because the molecule Tadalafil can remain in the blood for a longer period of time and can even build up in concentration in blood. If taken in small concentrations daily it builds up in blood up to a concentration of 20mg, necessary for sexual activity. It usually gets concentrated in the tissue of the penis and keeps the enzyme Phosphodiesterase 5 away and hence giving its therapeutic effects. Initially, this was associated with a risk with the drug but then it was recommended by the scientists to be taken in small doses daily which also waves off a lot of its side effects.

  • Effect of food :

Viagra is usually more affected by food than Tadalafil. Fatty foods especially hinder the absorption of Viagra and causing decreased levels to be reached in the blood that is usually insufficient to give a therapeutic effect. It is therefore recommended that it be taken on an empty stomach.

Grapefruit is a notorious fruit whose juice has numerous interactions with various drugs. Viagra also has interactions with grapefruit juice and its levels are usually increased to a toxic level in blood. Grapefruit juice hinders the clearance of drug and subsequently, it stays in the blood for a longer period of time, therefore, giving various side effects.

Alcohol should also not be taken with Viagra as it can also potentiate the side effects. Viagra and alcohol if taken together can cause toxic effects in the body. These two are also normally abused together. Viagra can cause some amount of dizziness and low blood pressure. So taken with alcohol it can cause a range of harmful effects.

Tadalafil is usually not affected by food at all. It can be taken with or without food. This is one more added benefit of ease that Tadalafil offers over Viagra.

  • Effect of Nitrates:

Both Viagra and Tadalafil are affected by Nitrates and are usually not recommended in Heart patients. The dose is adjusted in heart patients or they may be totally contraindicated and not prescribed to such patients at all. Nitrates cause a drop in blood pressure. Both Viagra and Tadalafil are also known to cause a drop in blood pressure. If taken together they can sometimes cause a fatal drop in blood pressure.

  • Contraindication in other health conditions:

Both Viagra and Tadalafil are sometimes contraindicated in Renal and Hepatic impaired patients. Sometimes depending upon individual condition the dose is adjusted. As we know that Liver and Kidney are two main organs that are responsible for clearance or elimination of the drug. They metabolise the drug and then eliminate it form body. If any problem occurs with their function then drug remains in the blood in its active form for a longer period of time and therefore gives side effects. So in patients who might be hepatic or renal-compromised the dose of Tadalafil and Viagra may be adjusted.

  • Side effects:

Side effects of both Sildenafil and Tadalafil are almost same. They include facial flushing, headache, back pains, sometimes indigestion and dizziness. Dizziness is most common with Viagra. But there’s one thing that should be noted that some men may find more side effects with one drug than the other. So it depends on the individual and their tolerance with the drug.

Conclusion (Which drug is better Tadalafil or Sildenafil):

The conclusion will always remain an individual choice. Based on the person, his age, his health condition, and how badly they are affected by their condition. But seeing above Tadalafil definitely has added benefits of ease of use over Sildenafil (Viagra). It has fewer food interactions and can be taken daily with a low dose. Its tolerance is more than Viagra. Side effects such as flushing and headaches are less observed with Tadalafil.

Difference between Vardenafil (Levitra) and Tadalafil (Cialis):

Vardenafil, brand name Levitra is also a drug used to treat erectile dysfunction. It belongs to same class of drug as Viagra, Cialis, and Spedra. Its mode of action is also similar to these drugs. It works by blocking Phosphodiesterase 5 enzyme in the blood and thereby treating erectile dysfunction. Let us compare Vardenafil and Tadalafil against each other and see the benefits that these medicine offer over one another.

  • Onset of Action and stay in blood:

Vardenafil onset of action starts within 30 min of its consumption. Its effects last up to 5 hours in blood. It is available in two dosage forms tablets and as a tablet that dissolves on the tongue. Staxyn (another brand name for Verdanafil) comes as a tablet that dissolves on the tongue. Its onset of action is 15 min and also remains in the blood for up to 5 hours.

Tadalafil onset of action is 30 min and remains in blood for up to 36 hours. The length of time it stays in your system is the longest compared to any other drug.

  • Daily dosage intervals and potency:

Vardenafil comes in doses of 2.5mg, 5mg, 10mg and 20mg. Just like Viagra it is also usually taken 30 min to one hour prior to a sexual activity. Like Viagra, it cannot be taken daily. It doesn’t accumulate in the blood like Tadalafil and therefore gives side effects. The most common side effect that it causes is Priapism (prolonged painful erections). The starting normal dose for Verdanafil is 10mg. 2.5mg and 5mg is the dose for patients who are renal or hepatic compromised. 20mg is the maximum dose and it cannot be increased than that. If the drug doesn’t suit an individual then a physician may seek to change the medication. If coitus is several times a week then Tadalafil may be a better option as it can be taken every day in a dose of 2.5mg.

The doses of Tadalafil are similar to Verdanafil but how they suit a particular individual depends upon patient condition and drug tolerability.

  • Effect of Food:

Vardenafil like Tadalafil is not as affected by food as Sildenafil (Viagra). Both Cialis and Vardenafil can be taken with food and their efficacy remains same but taking them with large fatty meals decreases their absorption. Grapefruit juice also has interactions with Verdanafil and it increases its concentration within the blood that results in unwanted effects of the drug.

Alcohol should also not be used with any of the erectile dysfunction drugs as it causes difficulty in achieving an erection and therefore the drug may not show its maximum effect.

Almost all of the erectile dysfunction drugs have the potential for abuse. They are usually abused with other recreational drugs or alcohol. But this has extremely harmful effects on the user and drug alone may not be able to treat the erectile dysfunction.

  • Contraindications with other health conditions:

Both Vardenafil and Tadalafil are contraindicated in Heart patients and Renal and hepatic patients. Almost all of the erectile dysfunction drugs are contraindicated in heart patients because of the drugs that they take. Heart patients are usually on Nitrates and these have a known drug interaction with almost all erectile dysfunction drugs. Nitrates and these drug lower patients’ blood pressure to a fatal level.

In renal and hepatic patients dose is always adjusted lowest dose is usually preferred.

One benefit that Verdanafil has over other drugs is that it is known to benefit erectile dysfunction in diabetic patients. The drug is well tolerated in diabetic patients.

One precaution should also be taken that if a person is on alpha-blockers for blood pressure or prostate problems then erectile dysfunction drugs should not be taken as they are known to cause a fatal drop in blood pressure with these.

Tadalafil is also prescribed for patients with prostate gland problems. A dose of 5mg is used for such patients.

  • Side effects:

Side effects of both the drugs Verdanafil and Tadalafil are almost the same such as flushing, blocked nose, headaches, vision problems at times. But Levitra is known to have lesser side effects than other drugs of the same class.

Is Tadalafil available without a prescription?

So far Tadalafil (Cialis) is not available without prescription like Viagra. However, the makers of the drug are working towards making it an over-the-counter drug this year. It is available on prescription. And the physician will check an individual, his risk factors, and his disease thoroughly before prescribing the medication.

CONCLUSION:

We have compared Tadalafil to various drugs of the same class to see why it should be chosen over the others. Choosing Tadalafil is purely based on individual choice and tolerability of the drug. If you’re still asking “why choose Tadalafil?” Tadalafil does provide with ease of use over other drugs as it can be taken daily with minimum side effects and coitus needs not to be planned around it. However, the best decision is always made by physician or pharmacist in choosing the right medication for you.

Drugs should always be a temporary solution to the problem especially to treat a condition like erectile dysfunction. They should not be the sole dependency for an individual where they ultimately get their confidence from. Erectile dysfunction is a condition that can be treated with the right diet, right lifestyle modifications, and sometimes treating underlying health conditions. Erectile dysfunction can sometimes act as a marker for the health of an individual and tells something deep about a person’s health – especially his heart health.  Drugs should not be the only solution to the problem as they ultimately cause psychological dependence for the person. The whole treatment of erectile dysfunction irrespective of the drug, whether someone is using Viagra, Tadalafil or Vardenafil, should be approached with positivity and appropriate drug usage as recommended by doctor or pharmacist.
